How To Use This Site
Thank you for visiting our site and participating in our online experience! This site may be new to you, so we're here to help you through the registration process and filling out your first form!
1. Register
To register for the site click on the Register link above.
You will be presented with the Register page:
Fill in your:
- Desired User Name (will check availability during sign-up)
- First Name
- Last Name
- Email address
- Requested password
2. Login
You will receive an email confirming your username and welcoming you. You will also be directed to the login screen.
3. My Forms
Once logged in, click on "My Forms" in the main menu. You will see several available forms that you may need to fill out:
4. Complete / Edit Submissions
On your selected form, if you have filled out the form previously, you will see a list of your previous submissions. You may view or edit existing submissions as needed.
On most forms, you may have the option to "Save and continue later" next to the submit button. If you need to come back later to finish, click this link, and you'll be emailed a link to continue when you're able.
